28. How do you format a flashback so the reader knows what's going on?

Flashbacks are always clearly signposted in the text."It all started three years ago when..." Sometimes, in addition I'll use past perfect constructions: "had been", "had said", "had whipped out his dagger before determining that they were a serious threat".
